  • Responsibilities

    The Community Health Aide is a paraprofessional employee, trained to provide personal care and related services in the community. This individual functions under the direction of a nurse supervisor.

     

    • Provide direct and indirect care elements to residents in a community under the direction of a nurse supervisor. May have to provide direct and indirect care elements to numerous residents per shift.
    • Demonstrate appropriate and safe techniques in personal hygiene.
    • Adhere to assessment/task sheets for community health aide.
    • Observe, report and document resident status according to Atria standards.    
    • Maintain a clean safe environment.
    • Assure adequate nutrition and fluid intake.
    • Safely transfer and ambulate resident.
    • Help maintain the resident’s physical and mental well-being.
    • Attend in-services as required.
    • Maintain CEU requirements in compliance with New York State law. Maintain home health certification.
    • Attend and complete various Atria-required trainings designed for community employees.
    • May perform other duties as needed and/or assigned.

     

    Qualifications

    • Completion of the 9th grade, prefer high school.
    • Able to read and write consistent with job.
    • Successful completion of a New York State approved home health aide training program (i.e. home health aide certification).        
    • Has a reliable means of transportation.
    • Meet health requirements.
